Hamilton, NJ: Primary Care (Behavioral Coming 2026)

What is an integrated care medical facility, and what makes Woods Primary and Behavioral Health Center at Hamilton unique?

At Woods Primary and Behavioral Health Center at Hamilton, we care for your whole health: your body and your mind. This is called integrated care. It means your doctors and behavioral/mental health providers work together as a team. They talk to each other, share information, and help you in the best way possible.

Our center is special because it is specifically designed for people with intellectual and developmental disabilities (IDD) and autism. We serve patients age 6 and older, with a particular emphasis on adults ages 18+, as individual transitioning from pediatric to adult care have a difficult time finding care.

At the Woods Primary and Behavioral Health Center at Hamilton, healthcare is tailored to each individual’s need in an environment. Wide halls and large rooms make it easy to navigate for people who use wheelchairs, and there is plenty of room for caregivers. It is also sensory-friendly, so it feels calm and safe.

Our providers understand how to care for people with IDD and autism. They speak clearly, include the patient in every part of care, and listen closely. Caregivers are welcome, too. We believe each person should be part of their own health journey.

What types of providers will you have on staff?

In our first year, we will have a primary care physician, a primary care nurse practitioner, psychiatric nurse practitioner, and a consulting psychiatrist. In our second year, we will add licensed clinical therapists.

Will this practice connect patients to specialists and, if so, how will that work?

Yes! This is a huge benefit to our center being located within a hospital, and one that is part of the outstanding RWJBarnabas Health network. We have a lab right in the office for basic lab work and testing. If you need an x-ray or a specialist (like a heart doctor or a skin doctor), we can help you see one right here on campus.

Our team will talk directly with the specialist’s once to set things up and make sure they understand your needs. We’ll also share information with them, so everyone is informed about your care.

If you need a specialist who is not part of RWJBarnabas Health, we can help with that too.

Will we still be able to use our existing providers?

You can still see your current specialists if you want to. We are happy to work with them to make sure you get the best care.

But for your main doctor, also called a Primary Care Provider (PCP), your insurance may ask you to choose one person or place. If your insurance requires that, you will need to pick our center as your main doctor to receive care.

Are there forms that can be sent in advance of the appointment that I can review/complete?

Once you register as a new patient, we will send a link to our patient portal, where you can complete forms electronically. Once filled out, they will be automatically sent back to us.

Will this practice replace urgent care? Are there same-day appointments?

Our center will provide sick visits and minor injury care Monday through Friday from 8:30 am to 5 pm and will offer same-day appointments based on availability, but we do not replace urgent care. After hours, our answering service can connect you with a provider, if necessary.

When will the Center first start seeing patients?

We will start seeing patients in Fall 2025. Please call (888) 628-9661 or email hamilton@woods.org.

We may need copies of prescriptions for caregivers due to our living situation. How will this be handled?

Our practice can provide paper prescriptions for most prescriptions. However, controlled drug substances have to be submitted electronically. We are also able to provide a medication list.

How can I become a patient/when can I make an appointment?

Please call (888) 628-9661 or email hamilton@woods.org.

Who can I contact with questions or concerns?

Ethan Diamond, our practice manager, will be able to answer most questions, or connect you with the right person.

Please call (888) 628-9661 or email hamilton@woods.org.

How do you accommodate sensory sensitivities and communication differences?

We understand that every individual experiences the world differently. Our adult and pediatric waiting areas are designed to be low-stimulus environments with space to move around, helping to reduce anxiety and sensory overload. Before the appointment, we provide tailored information to help patients know what to expect. Our approach ensures that every individual remains at the center of the experience.

Hamilton Hospital is big. How do I find the health center?

There is plenty of wayfinding and signage to make sure you easily find our health center. Park in Parking Lot D—you will see some designated spots for our center close to the building. Once parked, follow the signs to reach the Center, which is a short walk to Suite 501 in 1 Hamilton Place.
